- Liability Insurance payment for FY 2010-2011: March 31, 2010
- Audit Committee Report: August 2, 2010 (pages 40-48)
- Financial Report: August 2, 2010 (page 49)
- Financial Certification: August 2, 2010 (page 50)
- Updated List of Officers: August 2, 2010
- Name of you Booster Club Financial Institution and EIN#: August 2, 2010
The above forms and guidelines are available in the booster club manual which is available at my office or you can download the forms and/or the manual below. If you have any questions about completing any of these forms, please do not hesitate to contact me.
Failure to submit any of the required information will result in possible suspension of your Booster Club. Remember...YOU DO NOT WANT TO END UP ON MY "HIT LIST".
